The 508c1a Tax Codes are "special rules with respect to section 501(c)3"
This is why that we used to have to do the paper SS-4 form because they would automatically categorize us all under the 501c3 code but then manually had to enter a sub-category of the 508c1a for us because we are a mandatory exception for taxes as a sub-set of the greater non-profit category 501c3.
The one is not on donors to prove their donation was tax deductible- all they need is a receipt with who to, who from, amount/value/type of donation, and date of donation.
To be very specific about filling out the EIN fss-4 form, here are the fields which are required to be filled in to go into the correct category. If you have any questions, reach out to an advisor to review your form before you send it out.
The fields to fill in are:
1 - name of entity
3 - executor, care of name
4a, 4b, 6, 7a (same as 3)
8a is "no", 9a "church", 10 "banking purpose" Open bank account, 11, 16 "other" Church/Ministry, 17 "church", 18 "no",
Name (same as box 3), Founding Trustee, phone number, sign, date, fax number (optional)
